This Claude Code plugin enforces structured handoff discipline by injecting an SBAR/I-PASS-style [HANDOFF] block at the moment an agent is about to close a turn, ensuring decisions and next actions are explicitly stated rather than implicitly assumed. It scans the final message for transmission failures like unnamed decisions or missing handoff blocks, and flags them as non-blocking findings on the next prompt (or blocking with HANDMON_STRICT=1).
